File your complaint with the IRDAI: You can approach the IRDAI to register your complaint. You are also required to submit the necessary documents to support your complaint. IRDAI looks into all insurance grievance redressal matters and takes the necessary steps. 

Approach the Insurance Ombudsman: If no action is being taken against your registered complaint, you can escalate your issue with the Insurance Ombudsman. The Insurance Ombudsman tries to resolve the issue within 15 to 30 days of filing the complaint. 

Seek legal advice: If you suspect that the tampering was deliberate or fraudulent, you may need to seek legal advice to protect your rights and interests. For this, you can file your case in the civil court regarding the tampering of your insurance form or mis-selling of insurance policies.
